The three instruments

The MBI is the reference standard — 22 items, validated across hundreds of studies, normative data spanning four decades. Its weakness: it requires a licence fee per administration, and its three-subscale model (emotional exhaustion, depersonalisation, personal accomplishment) may underestimate burnout in practitioners who maintain high personal accomplishment scores despite being exhausted.

The BAT addresses this gap. Its four-subscale model adds cognitive impairment (concentration problems, memory lapses) and psychosomatic complaints (sleep disturbance, physical symptoms) — domains that the MBI misses entirely. It is free for non-commercial use, making it practical for clinics and training programmes.

The PFI was built for healthcare. Sixteen items, two scales — professional fulfilment and burnout — measured in parallel. This matters: burnout is not just the absence of fulfilment, and measuring both captures clinicians who are simultaneously burned out and still finding meaning in their work.

Choosing strategically

The guide recommends matching the tool to the purpose. For research or cross-study comparison: MBI. For comprehensive individual assessment: BAT. For quick organisational screening with actionable output: PFI. For most clinical training programmes and small practices: PFI is the pragmatic choice.

Three validated burnout tools — MBI, BAT, and PFI — serve different purposes. The guide helps organisations choose strategically instead of defaulting to the most familiar.

Limitations

Guide focuses on US healthcare context. Scoring thresholds may not apply across cultures without local validation. None of the three instruments captures organisational causes — they measure the syndrome, not the system.
